Over 45 and Still Playing? Why Golf Elbow Pain Lingers Longer After 45, and Why Waiting It Out Could Cost You Your Season

A sports PT who has treated tour players explains why the same swing hurts more as you get older, why every unprotected round makes it worse, and the one change that keeps you playing.

Golfer holding his elbow on the course

For golfers over 45, elbow pain often hangs around far longer than it used to.

At 35, a sore elbow was gone in a few days. Now it's been months.

⚠️ Golfer Warning
If you're over 45 and your elbow pain keeps coming back, every round you play without protecting the tendon can push the injury further along, and after 45 it takes far longer to reverse.

If you're over 45 and still playing regularly, you've probably noticed things aren't quite what they used to be.

You used to bounce back from anything. A sore elbow after a weekend of golf would be gone by Wednesday. Now it lingers for weeks. You rest it, it calms down, and one round later it's back.

You might have been told it's just your age. That you should play less. That this is what happens to golfers your age.

Sports physical therapist John Keller says that's the wrong conclusion.

Keller has spent 19 years treating golfers, including four players on the PGA Tour. A big share of the golfers he sees today are over 45.

"I've got golfers in their sixties and seventies playing four rounds a week without elbow pain," Keller says. "Age isn't what stops them. What stops golfers is not understanding how their tendons change, and carrying on with the same approach they used at 35."

Does This Sound Like You?

  • Your elbow pain takes weeks to settle, not days like it used to
  • You wake up stiff and sore the morning after a round
  • You've started skipping the range to "save" your elbow for the course
  • Gripping a jar lid, a kettle or a steering wheel now hurts too
  • You've wondered whether you'll have to give up golf, or cut right back

If two or more sound familiar, keep reading. There's a reason your elbow is struggling, and it's more fixable than you might think. But not if you leave it.

What Elbow Pain Is Really Costing You

Golfers rarely talk about it, but Keller hears the same things in his clinic every week.

It's standing on the 1st tee already bracing for the twinge. It's the round that started well, then falls apart on the back nine because every swing sends a jolt up your arm.

It's quietly shortening your backswing to protect the elbow, then watching your distance drop and your scores creep up. Keller sees this constantly: golfers changing their swing to protect a sore elbow, and picking up new aches in the wrist and shoulder as a result.

It's turning down a round with your mates because you "need to rest it." Lying awake because you rolled onto that arm. Wincing when you lift the kettle, carry the shopping, or pick up the grandkids.

And underneath all of it, the question most golfers over 45 don't say out loud:

"Is this the year I have to give up golf?"

"I hear that from patients all the time," says Keller. "And most of them didn't need to get anywhere near that point. They just waited too long, hoping it would sort itself out."

It's Not Your Swing. It's How Your Tendons Change After 45.

Every time you swing a golf club, one specific tendon at your elbow takes the brunt of the force. Research measuring this puts it at over 22lb of force with every swing.

That's the same whether you're 30 or 65. What changes is how well your tendons cope with it.

"As we get older, tendons get stiffer, they have less blood supply, and they rebuild damaged fibres more slowly," Keller explains. "So the same swing does the same damage, but your body takes longer to repair it."

Same force every swing+Slower repair after 45
= Pain that keeps coming back

Each swing creates tiny tears in the tendon. At 35, your body repaired them between rounds. After 45, the repair often isn't finished before your next round. So every round adds damage on top of damage that hasn't healed yet.

It's no coincidence that this type of elbow pain is most common in people in their 40s and 50s.

Why Waiting It Out Is the Riskiest Thing You Can Do

Most golfers over 45 handle elbow pain the same way: ignore it, play through it, and hope it goes away on its own.

Keller says that's exactly how a few weeks of soreness turns into a lost season.

1
Weeks: Irritation The tendon is overloaded and sore, but still healthy underneath. Take the load off now and recovery is usually quick.
2
Months: Breakdown Tiny tears build up faster than a slower-healing tendon can repair them. The pain keeps coming back and lasts longer each time.
3
A year or more: Long-term damage The tendon has been weakened over months. Recovery can take many months, sometimes a year or more, often with time off the course.

"After 45, everything in that timeline moves slower," says Keller. "The longer you leave it, the longer it takes to come back from. A golfer who acts at stage one might be fine in weeks. One who waits until stage three can lose a season, sometimes two."

And here's the part most golfers miss: if your pain has been coming back for more than a few weeks, you may already be past stage one.

Why the Usual Advice Doesn't Work Anymore

"Just rest it." Rest helps, but a tendon that repairs slowly needs far more than a week. Most golfers go back out before the tendon has recovered, and the cycle starts again.

"Wear a sleeve." A compression sleeve squeezes your whole forearm evenly. That can ease the soreness, so your elbow feels better. "But it doesn't change where the force goes," says Keller. "The same tendon keeps taking the full load of every swing. You just feel it less, which is the worst combination when your tendons repair slowly."

Here's the problem with a sleeve: it squeezes everything, so it can't target anything.

A compression sleeve spreads its pressure evenly around your whole forearm. But golf elbow pain isn't a whole-forearm problem. It comes from one specific damaged tendon.

Squeezing everything a little can't put focused pressure on that one point. So nothing changes where the force of your swing goes, and the damaged tendon keeps taking the full hit, swing after swing.

Compression sleeve compared with the injured elbow tendon

A compression sleeve squeezes the whole forearm, not the one tendon that's actually injured.

"Play less." This is the one Keller pushes back on hardest.

"Golf keeps a lot of my older patients active, social and healthy. Telling them to stop is the last thing I want to do." John Keller, Sports Physical Therapist

"The answer isn't playing less," he says. "It's protecting the tendon while you play, so it finally gets the time it needs to heal."

What Keller Has His Older Golfers Do Instead

For tendon pain like this, sports medicine has long used an approach called counterforce bracing.

Instead of squeezing everything, a counterforce brace applies targeted pressure to one point on your forearm, just below the injured tendon:

  • The tendon is the weak point. And after 45, it's slower to recover from repeated force.
  • The muscle below it is built for load. Muscle is designed to take force.
  • The pad sits right on that muscle. When you swing, it catches the force first and pushes it into the muscle, so far less of it reaches the damaged tendon.

A compression sleeve squeezes the whole arm. Counterforce redirects the force away from the tendon.

"For golfers over 45, this matters even more," Keller says. "If your tendon repairs slowly, the best thing you can do is stop loading it every swing. That's what gives it the chance to catch up."

The Brace Keller Recommends

Not every brace labelled "counterforce" is built like the ones used in the research. Many are thin straps with no defined pressure point.

"I could only find one brand that uses the exact counterforce design used in the studies," says Keller. "That's Control Point Pro. It's what I recommend to all my golfers, whatever their age."

Control Point Pro counterforce elbow brace

Control Point Pro: a precise counterforce pad that targets one point, instead of squeezing your whole forearm.

  • Precision counterforce pad: takes force off the weak point with every swing.
  • Simple to put on: one strap, no fiddly fastenings. On before the first tee, no adjusting between holes.
  • Low profile: sits flat enough that you forget it's there mid-swing, with no bulk catching your sleeve.
  • Stays put: EVA grip padding stops it sliding down your arm, even on hot days.

"What About Strengthening Exercises?"

Keller is a big believer in strengthening, especially after 45. "But building strength takes longer as we age too," he says. "That can be months. In the meantime, the tendon still needs protecting from the force of every swing, or you're re-injuring it faster than the exercises can help."

His advice: do both. Strengthen over time, and wear Control Point Pro every time you play.
